I have not been diagnosed with any sort of condition but I have had the same issues that you have had. What I found works best is the tightest fitting undewear you can buy and a harness with adjustable leg loops. I had to cut the elastic off the my petzl adjama to make sure the leg loops fit tight enough.

I started wearing Hanes with the special pocket and they are the most comfortable underwear I have for climbing. Try to keep everything as tight as you can.

Skip tightest fitting unless you want a monster headache. Go for something like Saxx which keeps all the eggs safely nestled out of harms way. 

I found the misty mountain Cadillac helped compared to my old petzl harness with a similar issue.

I work in harnessed aerial arts and we call the organization of the goods to be able to dance without causing pain / damage, "the junk show".
Best solution is a jock strap (best) or tight fitting underwear that puts it all front and center in a nice little package and keeps things out of the way of the leg loops.

Guideline #1: Don't be a jerk.
