Upper arm pain close to the elbow

Hi,

I've been having pain in my upper arm for roughly 18 months now and have been to both doctors and PT's multiple times, however, nothing has fixed the pain. So I thought I'd ask here if someone else has had similar issues.

It started in the spring of 2019 when I was pushing myself alot in climbing and in the gym. There was no acute pain but it slowly built up and I thought it was due to overuseage. The pain slowly faded after a long rest but got really bad after a trip to Fontainebleau that atumn. It has gotten a bit better but never good since that. This was almost a year ago. 

The pain is mostly a dull pain in the lower region of my upper arm as I marked on the picture below which is gradualy increasing after load.  
A normal day I can just barely feel the pain. But when starting to load the arm more than ~70% I slowly start to notice it. It then gets worse during a few hours and can stay on for days/weeks.

I've preciously done MRI on the elbow and neck. No compressed discs and all tendons in the forearm and distal biceps/brachialis looks fine.
I've also done an EMG which found no problems.

I recently did an MRI of the shoulder it showed a very small SLAP-tear in the shoulder. I immediately went out to throw a ball to test this. Which gave me the same symptom in the upper arm as I get from lifting/climbing. 

Have any of you experianced anything similar?
Could it be possible to get pain in the lower part of the upper arm from a SLAP injury in the shoulder?
According to my doctor it is highly unlikely but my PT was very convinced this was the problem.

Do your symptoms match medial epicondylitis?   The pain points on your drawing are a pretty close fit.  

Guideline #1: Don't be a jerk.
