Maryanne Gonzalez at Select Physical Therapy (Henderson location, I think). I don't know what your specific condition is, but she gave me both treatments and exercise prescriptions that were helpful for my shoulders. She'd probably appreciate the novelty of treating another climber (besides me) instead of yet another worker's compensation case.
Donald Ozello (Championship Chiropractic) is very thorough, not a fraud, and is also well connected in local athletic and athletic training circles; he can try to treat your pain and may know someone else to recommend for physical therapy. He also has a lot of rehabilitation and prevention exercises posted on Youtube.
I went to Bob Donatelli. He wrote (or maybe edited) a textbook called "Physical Therapy of the Shoulder, 5th edition." He has worked with a ton of pro athletes.
