Planet Claire may actually be the hardest climb by grade in New York at 14b.

I'm pretty sure there's some sport climbs in the 13d/14a range at Hidden Valley in VA. Somebody with better beta than me might be able to give you more specifics.

Southern Comfort in the Concave is 5.14a. I know there are other hard ones around both at LRC and PB.

The Madrigal was a long time project recently sent in Alabama (14a), and I believe Jimmy Webb recently established Cookie Monster (14-) in Tennessee, which is the new hardest route in the state.

Jeff Shapiro did a route at Kootenai, Relativity, which he also called 13d I believe.

Golden Ticket (RRG, KY) is considered 14c, not 14d.

CT's hardest route from my understanding is over in the Eastern part of the state at a small crag that's on private property and is posted. Think it was done in the 80's or 90's.

Sprague... I think you may have been to this one? Do you by chance know the area I'm referring to and the name of the route?
