Here is a firsthand account of a death believed to be caused by a gearsling.

But seriously, if this is what you're worried about, I suggest you take up a different sport. This 1 in a billion type accident is simply not a good reason to not use a sling.

I agree. Really sad story, but the odds on that one are astronomical. For sure, many thousands more people per decade win the lottery than experience an accident like this. For reference, more people get lucky with Eva Mendes every year than have an accident like this per decade, from the sounds of it. If this kind of threat stresses you out, it's definitely time to choose more indoor pursuits.

Afterthought: Maybe this makes a case for the double-D attachment on the Metolius gear slings, which if used by the leader discussed in the link, might have limited the shifting/impact to the neck in this incident? I like the D-D for its increased sling capacity as well as making the gear sling sit much better and not shift.

I have personally had to get a partner off a route where he took a fall and got an over-the-shoulder sling caught on a tree and was being strangled. Yes, left there, he would have died. As it was, we got him to a hospital for some stitches and treatment of a concussion. It was ~1992 and on the climb Horny Bitch in the Red River Gorge.
