There are several lessons to be learned from this incident. |

Very preventable. How many people have to die from rapelling before people learn that they aren't special and that shit does happen. Use a damn friction hitch and put knots in the end of your rope. Tragic. |

Did his rappel device slip out the rope's ends, or the anchor failed?He rappelled off the end of the rope. The anchor did not fail. The media misreported it. |
