Long story short: a climber rappelled off the end of his rope on Frosted Flake and died.

Here's an article from the local press. As usual, the accident details are vaguely reported. But it does help us remember the victim, which is the main point anyway.

Wow, that's terrible news. Condolences to the climber's loved ones.

As Sean Cobourn pointed out, the tragic part is how preventable this accident was. Hopefully, other climbers can take this to heart.

There are several lessons to be learned from this incident.
1. communicate with your partner! Are both ends down?
2. tie knots in the ends of ropes when rappelling in case your partner can't hear you. You can't rap off the end of your rope if it won't go through your device.
3. wearing a helmet is a great idea.
4. be an organ donor. five people got a new lease on life thanks to this climber's foresight.

Very preventable. How many people have to die from rapelling before people learn that they aren't special and that shit does happen. Use a damn friction hitch and put knots in the end of your rope. Tragic.

"An avid climber, Haddock was rappelling down a cliff when his rope came loose from a climbing pin about 10 a.m., Rutherford Sheriff's Detective Billy Scoggins said. Rescuers estimated he fell 30-40 feet."

What does it mean by "came loose from a climbing pin"? Did his rappel device slip out the rope's ends, or the anchor failed?

He rappelled off the end of the rope. The anchor did not fail. The media misreported it.
Guideline #1: Don't be a jerk.
