A friend and past client of mine, Murad Ashurly, just fell to his death on Ama Dablam. Not much beta available right now, but he was descending from a successful summit (unguided) when a fixed line failed and he fell 400 feet.

Forgot to clip one of the backup lines? The guy who rapped just before me on this section also broke his rap line, but only fell 10 feet or so to the next anchor (and his pack broke most of the fall).

Tragic regardless, my condolences.

Sorry to hear about the accident and loss of your friend. I recall the lines on that peak being less-than-inspiring; while we were up there a friend and partner broke one of the rap lines - thankfully he was backed up on a second one.
