Access Issue: Loose

The rock is so loose! A massive piece cracked off and landed on my belayers leg causing her to need immediate surgery. The EMT that arrived told me they'd been called for this wall in particular 3 times in the last 3 weeks due to rock coming loose.
Description
This route starts with some easy 5.10 climbing past 3 bolts up to a small roof. Pulling through the roof is not obvious and feels around V6; and clipping the 4th bolt is very strenuous. Once established above the roof about 20 feet of technical 5.10 climbing in a fun dihedral guards the chains.
