Assuming the biner was in fact not properly locked, as seems to be the consensus here, it is another reminder of the importance of having the belayer and climber check not only their own gear before initiating a climb, but also each others.
No one leaves the ground with me belaying unless I've checked their harness and their tie-in; in large part because I don't want to be responsible for something like this.
Don't know if that would have made the difference here, but it certainly reduces the likelihood of such events.
